Medical information

  • Information about any prescribed or non-prescribed medication you are taking

  • If you have it, your latest prescription from your optician

You will be asked to complete our registration form, so please have to hand details of your

  • General practitioner (GP)
  • Optician

Payment method

If you are paying using medical insurance, please bring your membership number and authorisation code.

If you are self-paying then please bring a credit or debit card.

On the day of your visit

Please arrive at the location 15 minutes before the time specified in your appointment letter. This will help to any avoid delays.

If this is your first appointment, you will be asked to confirm your personal information to ensure that the registration details we have for you on file are correct.

You will also be required to complete an “undertaking to pay” form, even if your account is being settled by a third party. If you are covered by private medical insurance, please bring the name and address of your insurer, your policy number and authorisation code with you.

Diagnostic tests

Most patients will have some diagnostic tests during their outpatient visit to Moorfields.

Please note that some tests require dilation. This is where some eye drops are administered to widen your eyes and allow the doctor to better see the back of your eyes.

Your appointment letter should indicate whether there is a possibility of dilation. We recommend that you:

  • Do not drive to their appointment as you will not be able to drive for up to 6 hours after dilation
  • Bring sunglasses with you as you will be more sensitive to light after being dilated


Moorfields Private Pharmacy is a registered pharmacy. We provide a comprehensive ophthalmic dispensing service to all private patients using our inpatient and outpatient facilities and for patients with private prescriptions from the community.

Our opening hours are Monday to Friday 9am - 7pm.

On departure

Before you are discharged we will provide full details about any ongoing medication or treatment you may require. Any take home medications will be dispensed by our private patient pharmacy.
